Polyclinic therapy with a course of AFP (family medicine)
Department of Polyclinic Therapy at Kuban State Medical Institute named after.The Red Army was organized in 1987, which until 2009 was led by Doctor of Medical Sciences, Professor Vasily Vasilievich Kolesnikov.From 1998 to 2009, Professor V.V.Kolesnikov headed the regional branch of the Association of General Practitioners.
During the formation of the department in the 6th year of the Faculty of Medicine, scientific and pedagogical activities were carried out by: head.Department Associate Professor V.V.Kolesnikov, candidates of medical sciences.Sciences G.A.Golubtsova, B.A.Yarysh, A.Ya.Dzharimov, assistant V.P.Shirokova.In 1996, on the initiative of Professor V.V.Kolesnikov at KSMU, a training and treatment module “Outpatient clinic of a general practitioner (family doctor)” was created, which served as the prototype for the currently operating offices of a general practitioner (GP).From 2001 to 2006 at the department under the leadership of Associate Professor A.S.Bagdasaryan, a postgraduate training course was organized in the specialty “Emergency Medical Care”, which in 2006 was allocated to an independent department of FPK and teaching staff.In 1998, the department initiated an initial six-month training course for doctors in the specialty “General Medical Practice (Family Medicine).”A logical continuation of the progressive development was the creation in 2006 of the course “General Medical Practice (Family Medicine)” as part of the Department of Polyclinic Therapy and the opening of a clinical residency.
Since 2009, the department has been headed by Vitaly V. Gorban, MD, a lecturer with 35 years of teaching experience, a scientist whose interests include morphofunctional and microcirculatory features of the gastrointestinal tract, regulation of the autonomic nervous system, diagnosis and prevention of risk factors in comorbid pathology.
Over the years of the department's existence, the staff of the teaching staff has undergone changes, teachers have been replaced by their students.
By all parameters, polyclinic therapy as an academic discipline is a pre-graduate stage of medical activity and students of 4-6 courses of the Faculty of Medicine in the specialties "polyclinic therapy" and "medical rehabilitation" study at the graduate department. The educational bases of the department are city polyclinics No. 3, 7, 10, 11, SKAL KKB polyclinic No. 2, outpatient polyclinic department for the attached population of Krasnodar GBUZ "Scientific Research Institute– Regional Clinical Hospital named after S.V. Ochapovsky", polyclinic department of CHUZ "Clinical hospital "Russian Railways-Medicine" of Krasnodar".
The practice of "Assistant physician of an outpatient clinic" in the 5th year of the Faculty of Medicine is carried out in all polyclinics in Krasnodar and the Krasnodar Territory. It is outpatient training that allows students to "feel like doctors" for the first time, to see not the declaration, but the implementation of an individual approach to the patient, the formation of medical logic and participation in the therapeutic and diagnostic process under the guidance of experienced mentors.
At the Faculty of Advanced Training, the expansion of educational and methodological work was manifested in the initiation and organization of such thematic improvement cycles as "Formation of a healthy lifestyle", "Medical care for tobacco cessation and treatment of smokers", "Prevention of chronic non-communicable diseases".
The priority development of primary health care in the modernization of Russian healthcare initiated the work of the department on the introduction of forms of distance learning (DO) in the framework of continuing medical education (CME). Cycles of primary training (PP) and advanced training (PC) were organized in various sections of general medical practice (GP) and therapy with an emphasis on improving dispensary supervision, organizing 3 stages of rehabilitation measures based on day hospitals, providing palliative care to patients, and organizing patient-centered admission in a modern polyclinic with a focus on on cancer awareness, based on the principles of "Lean manufacturing".
An important area of the department's activity is working with clinical residents in the following specialties: "polyclinic therapy" and "general medical practice", as well as with graduate students.
The scientific work of the department, based on comprehensive research, is focused on improving the effectiveness of diagnosis, prevention and correction of universal modifiable risk factors for the development of comorbid somatic pathology in outpatient practice for their subsequent targeted correction. This topic is a priority for the research work of the department staff and graduate students. In 2017, the staff of the department took an active part in the scientific study of ESSE-RF-2 "Epidemiology of cardiovascular diseases in the regions of the Russian Federation". The results of the scientific activity of the department since 2019 are reflected in 54 publications, of which 12 are articles in journals recommended by the Higher Attestation Commission for the defense of dissertations for the degrees of candidate and Doctor of Medical Sciences. Every year, the department organizes city/regional scientific and practical conferences on topical issues of comorbid pathology in outpatient practice for AFP doctors and internists in the framework of R&D.
The department is responsible for conducting an internship in the specialty "Assistant physician of an outpatient clinic" in the 5th year of the Faculty of Medicine, which is carried out in all polyclinics in Krasnodar and the Krasnodar Territory.
All the activities of the department are aimed at the final result – the restructuring of primary medical care in the Krasnodar Territory and the improvement of medical care for the population.
